Date: Every Thursday afternoon
Time: 12 to 4pm, alternating between two locations (so be sure to confirm the location before you register/arrive)

Commack: Stony Brook Advanced Specialty Care, 500 Comack Road

What to expect:
- Participants will complete questionnaires ahead of time and schedule a 30- to 45-minute visit.
- The screening will not provide a diagnosis but may provide information you can use in follow up with your health care providers.
- At the conclusion of the screening, you'll be provided with helpful resources as well as information about participating in ongoing research studies at Stony Brook University.
